​Case Manager

​Full-time with benefits -  case manager needed for SSG Fox Suicide Prevention program - position entails providing case management services to Veterans in  Franklin County, must have a driver's license, ability to engage and work with Veterans, and basic computer skills for documentation of services. Peer specialist

Part Time - Per diem, flexible hours, Veteran position to assist with outreach to other veterans, provide transportation and support, assist with HBA community events, and participate in retreats. Outreach Workers – Per diem – Preferably Veterans

Part Time - Per diem: Homeward Bound is hiring per diem outreach workers to work for our newly awarded grant – the SSG Fox Suicide Prevention Grant. This is a three year grant with the possibility of ongoing continuation. Outreach workers will be working on a per diem basis and able to work remotely. We are looking for outreach workers in multiple counties in the region. Job requirements will be outreaching to Veterans struggling with isolation, suicidal ideation, depression etc. Phone and in person visits will be required. Ability to assist Veterans in completing simple screenings and assessments for suicidality also required. Transportation may be a part of the job with mileage reimbursed. Assistance with recruiting with HBA’s retreats for PTSD is large part of the job and working the retreats is also a possibility. This is a flexible position with the ability to set your own hours and work from home. Some in person meetings and trainings in Saranac Lake with be required. Must be able to drive (Valid NY State License Required). Ongoing supervision will be provided as well as collaboration with other Veterans. This is an hourly per diem position - $20.00 an hour with mileage reimbursed. Consideration will be given to a full time position if interested. Submit resume and cover letter below.
